Enhance primary care initiatives as a Quality Improvement Decision Support Specialist. Drive data management and support eReferral implementation while collaborating with Family Health Teams across the region.
This role focuses on data management, reporting, and implementing digital health tools. You will collect and organize data to support quality initiatives, develop EMR tools, and maintain communication with EMR providers. Collaborating closely with the QIDSS Lead, you will also assist with onboarding eReferral workflows and provide training for clinicians.
Key Responsibilities:
• Collect and organize data from the EMR
• Develop and maintain usability tools for data
• Create and distribute user-friendly performance reports
• Support eReferral onboarding and training for FHTs
• Monitor adoption metrics and troubleshoot workflow issues
Requirements:
• Post-Secondary degree required
• Experience in health informatics or management preferred
• Understanding of primary care delivery
• Familiarity with digital health tools is an asset
• Willingness to travel to remote locations
