Pay Range
Base Salary: $61,000 – $76,000 (Commensurate with Experience)
Care Team Supervisor (CTS)
The Care Team Supervisor (CTS) is a fundamental role that provides essential leadership and oversight of a dedicated geographical team of PSWs and Care Team Scheduling Coordinators ensuring to provide top‑notch care and support to our clients. CTS responds to escalated client or PSW issues that may involve joint visits with clients, families, and funders. The role combines both nursing clinical expertise and operational/systems proficiency to enable positive outcomes for both clients, service providers, office staff and funders.
Key Areas of Accountability
- Lead a team of Personal Support Workers, Personal Support Coaches, and Care Team Scheduling Coordinators.
- Accountable for all performance‑management activities including performance appraisals, coaching, mentoring, and developing staff. Implements progressive disciplinary action as required.
- Listen and address clients and families’ concerns and convey understanding and commitment to explore solutions and options.
- Monitor operational performance and key performance indicators to assist with operational decision making.
- Support QI projects and process improvements.
Required Qualifications
- RN or RPN with CNO registration in good standing.
- Membership in skilled associations an asset (RNAO, RPNAO, CNA Certification).
- Minimum of 3 years experience in community health preferred.
- 2–3 years of supervisor experience in health care strongly preferred.
- Knowledge of the Occupational Health and Safety Act, relevant government legislation including Employment Standards Act, the Ontario Human Rights Code, PHIPA.
- Computer literacy (Power BI) and Microsoft Office application skills required.
- Familiarity with GoldCare client management program an asset.
- Proven skill with data‑informed decision making and report analysis.
- Valid driver’s license and use of a car on the job is a requirement.
- Weekend and after‑hours commitments are required.
