Key Responsibilities:
• Perform detailed in-home health evaluations
• Facilitate medication and health history reviews

• Offer preventive diagnostic services, as necessary
• Conduct virtual visits as required in care pathway
Requirements:
• Active state licensure in applicable areas
• Certification in relevant clinical practice required
• Preferred primary residence in VA or neighboring states
• Comfort in diverse home settings and environments
• Driven to enhance patient care and community health
