Salary: $53.09 – $66.36 per hour (To commensurate with experience and consistent with UHN compensation policy)
Hours: 37.5 hours per week
Shifts: Weekdays

Role Description
The Acute Care Nurse Practitioner, Substance Use Services is a specialty-based advanced practice nurse functioning in an expanded clinical role with a high degree of autonomy, expert clinical judgment, and leadership. The NP provides comprehensive, evidence-informed care to complex tertiary-level patients with substance use disorders and co‑occurring medical, psychiatric, and psychosocial conditions across the continuum of care. The role integrates direct comprehensive clinical care, quality improvement, organisational leadership, research, education, and professional leadership to advance excellence in substance use care delivery at UHN.
The NP acts as a clinical expert, consultant, educator, and leader within the interprofessional team and across the organisation, contributing to innovation, policy development, and system‑level improvement in addiction medicine.
Specifically, the NP will play a critical leadership role within UHN’s new Substance Use Disorder Integrated Care Pathways (SUD ICPs), dedicated teams that aim to provide patients who use substances that present to UHN’s emergency departments with dedicated substance use care,
as well as connection to services and supports provided by community partner organisations including the Centre for Addiction and Mental Health (CAMH) and The Neighbourhood Group (TNG). The NP will also provide medical supports to the Substance Use Intervention Team (SUIT), an Addiction Consult Service available across UHN’s inpatient and other clinical areas that provides addiction medicine support to reduce self‑discharge related to substance use needs. The NP will deliver care collaboratively within interdisciplinary teams of healthcare professionals, including physicians, nurses, community health workers, and peer support workers.
Qualifications
- Completion of a recognised Masters degree programme in Nursing
- Current certificate of (licensure)/Current registration in the College of Nurses of Ontario’s Extended Class
- Basic Cardiac Life Support (BCLS) Level II: Basic Rescuer – required
- Mental health subspecialty certification (e.g., CACCF, CNA CPMHN(C), Concurrent Disorders, ABA, PIECES) – an asset
- 3–5 years clinical experience in Mental Health & Addictions with marginalised/underserved populations – an asset
- 2–5 years clinical experience in medicine (e.g., Internal Medicine, Emergency medicine or medical subspecialty) – preferred
- Experience in a community health environment - an asset
- Advanced Practice Provider experience - an asset
- Current membership in RNAO (Registered Nurses Association of Ontario) is preferred
- Current membership in a related professional interest group or association is preferred
Knowledge & Expertise
- Standards of mental health care across multiple health disciplines
- Demonstrated understanding of the Mental Health Act
- Demonstrated understanding of the social determinants of health, intersectionality, and harm reduction philosophy
- Exceptional understanding of trauma‑informed and culturally safe care
