Role Description

The Support Coordinator 1 for the CAPCC Program will be responsible for helping individuals under the age of 65 living in Facility‑Based Continuing Care centers to access community supports to address social isolation and loneliness, removing barriers to community access and social inclusion for the individuals. Candidate will deliver client‑centered social support and case‑management services. This includes assisting clients with service coordination, conducting needs assessments, maintaining accurate documentation, and attending activities in the community with the individuals. Effective communication with clients, colleagues, and other service providers will be a key part of the role. This is a full‑time,



on‑site position based in Red Deer, AB, and serving other rural locations.

Qualifications

- Strong Interpersonal Skills and Communication abilities to build relationships and collaborate with clients, families, and staff
- Must have a class 5 license and reliable vehicle – as position involves a lot of travel
- Must transport individuals in personal vehicle
- Lifting and transferring wheelchairs, walkers, oxygen tanks etc into vehicles
- Positive verbal communication – position involves interactions with individuals with various needs/disabilities and external stakeholders
- Positive attitude a must, eager to support and take initiative to help individuals with community engagement goals
- Experience in Case Management and an ability to assess, plan, and facilitate appropriate interventions and support plans
- Skilled in Person Centred support to address client needs effectively and provide guidance on services
- Organizational skills with a focus on accurate documentation and adherence to policies and procedures
- Knowledge of social services programs and community resources in the area is an asset
- Ability to maintain confidentiality and work within ethical guidelines
- Post‑secondary education in Social Work, Human Services, or a related field is required.
