In this role, you'll engage in comprehensive nutrition assessments and use your skills in motivational interviewing and group facilitation. Your expertise will support patients in managing conditions including diabetes, obesity, and cardiovascular disease. Effective communication with the primary care team is essential, ensuring that nutritional interventions are monitored and evaluated regularly.
Key Responsibilities:
• Conduct individual nutrition assessments to identify health concerns
• Develop tailored nutrition care plans for chronic disease management
• Facilitate group sessions to enhance patient access to nutrition
• Maintain accurate client records and monitor progress
• Collaborate with interdisciplinary teams to improve nutrition services
Requirements:
• Bachelor’s degree in Nutritional Sciences or equivalent
• Minimum 2–3 years experience in community or clinical settings
• Good standing with relevant regulatory body
• Excellent interpersonal and counseling skills
• Ability to maintain confidentiality and work collaboratively
