Transform patient care as a Nurse Practitioner in a cutting-edge Hospital at Home initiative. Provide vital acute care to adults in their homes while acting as the Most Responsible Provider.
This dynamic full-time role involves working within an interprofessional team to ensure effective patient outcomes. You will deliver acute care, coordinate clinical decisions, and utilize PowerChart for documentation and care coordination. Mentorship is available to support your transition into acute care, with flexibility in scheduling as the team grows.
Key Responsibilities:
• Provide in-home acute care to adult patients
• Coordinate care as the MRP for your patients
• Collaborate with nursing, pharmacy, and rehabilitation




• Document care through PowerChart EMR
• Participate in rotating 12-hour shifts to sustain coverage
Requirements:
• Master’s level NP program completion approved by BCCNM
• Eligibility for registration as NP with BCCNM
• Solid collaboration and communication skills
• Skill in applying health research and evidence-based practices
• Ability to lead and evaluate healthcare teams
