Key Responsibilities:
• Manage compassionate care for a select group of patients
• Build and nurture strong patient relationships




• Use same-day testing data for effective care guidance
• Develop treatment plans with a supportive referral structure
• Engage in continuous education with colleagues
Requirements:
• Valid registration with the College of Physicians
• OHIP billing number required
• Necessary qualifications: CCFP, CCFP (EM), or FRCPC
• Preventive care experience is beneficial
• Commitment to at least 3 clinic days weekly
