This posting will be used to fill 1 permanent full-time position working 60 hours bi-weekly. It will establish a 12-month eligibility list which may be used to fill similar positions.

This job is based in the thriving and diverse city of Whitehorse in the beautiful Yukon.

Your role

The Social Worker provides professional social work services to residents/clients in Home Care within the scope of practice, standards of practice, organizational policies and procedures, and competencies.





The responsibilities for this position include working in an interdisciplinary team environment to provide psychosocial assessments and social work services for all clients and their families. This can include providing care coordination, support, responding to issues of abuse, neglect & self-neglect, and advocacy support for clients of varying complexity. You may also provide education & support to palliative clients, and their families, including advanced care planning and end of life planning.

Essential qualifications

- Bachelor of Social Work
- Current registration or eligible to be registered with a Canadian Social Worker regulatory body
- Recent experience providing case management as a Social Worker
Recent experience completing assessments

Conditions of employment

You must meet the following conditions before being hired, or within the time frame noted.

- RCMP Vulnerable Sector Check
- TB Screening
- CPR Level C with AED (completed within the last 30 months)
- WHMIS 2015
- Valid Class 5 driver’s license
- Current registration with a Canadian Social Worker regulatory body

Job requirements

This job requires you to perform, and/or be exposed to, the following:

- Exposure to pets, second-hand smoke, latex products, flowers and/or plants, bed bugs and/or other pests, and other irritants while in client’s homes or within facilities
- Travel within Whitehorse and surrounding areas, in possible inclement weather, is required
