This role enables you to deliver patient education, facilitate care coordination, and contribute significantly to quality support. You will help bridge care gaps and enhance the overall health outcomes of your patients through diligent assessments and creative patient engagement strategies. Key Responsibilities:

- Conduct in-home health evaluations for members
- Perform Annual Wellness Visits and medical assessments
- Deliver preventive health education and counseling
- Identify diagnoses for care management documentation
- Maintain accurate EMR documentation for compliance Requirements:
- Valid NP License in the working state
- 3 years patient care experience preferred
- Previous risk assessment experience is a plus
- Reliable transportation for home visits
- Ability to work independently and within EMR systems Utilize your expertise to support patient health and well-being through proactive clinical engagement and comprehensive assessments.
