ROLE PURPOSEThis position will provide outreach support to client(s) referred to Connective by our funders. You will work to enable clients’ adjustment to and success with life in the community.STATUS AND CLASSIFICATIONUnionized. REPORTING AND WORKING RELATIONSHIPS Reports and is accountable to the Program Manager or their designate.nParticipates as a member of Connective team to ensure quality and continuity of services to clients.nLiaises with staff and volunteers of community and government agencies as required.nLiaises with Connective staff as necessary.nAccepts clinical direction from the care team.nParticipates as a member of Connective staff team. KEY ACCOUNTABILITIES Receives referrals from funders.nWorks with the client, researches background material, and seeks the input of other professionals and family members,
to provide the assessment that will ascertain needs and postulate intervention possibilities.nWorks with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals.nEnsures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this plan.nEnsures the client’s ongoing legal, mental, and physical health issues are addressed in the plan.nWhen and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goals.nInteracts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals.nOn the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services.nMaintains extensive records and case notes in accord with Community Living British Columbia policies and standards.nCompletes monthly progress reports.nParticipates in staff and resident/staff meetings as instructed by the Program Manager.nOther duties as subsequently negotiated. WORKING CONDITIONS Functions independently,
while remaining a crucial member of the care team.nAdheres to Connective policies and procedures.nIs available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules.nIs a member of CUPE Local 1936-11.nReceives benefits and works within the working conditions as outlined in the collective agreement and personnel manual. EXPERIENCE AND SKILLS Graduate with a Diploma or Certificate in Social Services and one-year of relevant experience is preferred; inhouse training can be provided.nCounselling experience in the appropriate field and demonstrate a clear ability to respond well in crisis situations.nExperience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders.nUnderstanding of providing service to people with different learning abilities.nExtensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvement.nExperience in assessment, case/care plan development and implementation.nDemonstrated an ability to deal with potential violence.nDemonstrated an ability to develop programs tailored to the particular diverse needs of the individual.nExcellent written and verbal communication skills in English. REQUIREMENTS Ability to successfully complete a Criminal Records Check.nCurrent Emergency First Aid Certificate.nClass 5 driver’s license.nPersonal transportation that is in valuable repair, insured for business up to $1,000,000 for liability, and which can be used to transport clients. COMPENSATIONAs Connective is a unionized environment, the starting wage is $27.54 per hour. The wage will increase with every 2,000 hours worked up to a maximum of $31.61 per hour.
