Your goal in this position is to provide excellent patient and family-driven care, in a supportive and culturally sensitive manner. You will do this by collaborating with an interdisciplinary team of care providers, with an emphasis on the assessment, education, prevention, maintenance, and restoration of patient health. At North York General Hospital, we continue to advocate for healthcare initiatives in the diverse community we serve. North York General Hospital’s (NYGH) Reactivation Care Centre (RCC) is an interprofessional, collaborative care model designed for patients who no longer require hospital care but need restorative care to support their transition home or to another setting,



like long-term care. North York General’s RCC is located at Finch Site (formerly known as Branson) at 555 Finch Ave West, a newly revitalized space for patients, caregivers, staff & physicians.

In December 2017, the Ministry of Health and Long Term Care (MOHLTC), in partnership with multiple hospitals in the Greater Toronto Area, opened the Reactivation Care Centre – A Central LHIN Hospital Collaborative. North York General is one of the occupying hospitals at this site and as such, we are hiring Registered Nurses to work in our Alternative Level of Care (ALC) unit.
