This Social Worker II position, based at the Cold Lake Community Health Services, provides social work services as part of inter-disciplinary team to clients in acute care, continuing care and the community homecare settings at the Cold Lake Community Health Services and Cold Lake Healthcare Centre sites.


As a Social Worker II,

you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

As required.

Preferred Qualifications:

One year experience with acute care, continuing care and/or homecare. Enhanced knowledge of community based supports and services. Experience and knowledge related to AHS system navigation, placement and transitions in care and financial supports.
