The Single Independent Living Support Worker works as a member of the Wellness Integrated Support Homes (WISH) under the direction of a healthcare professional (Occupational Therapist, Nurse, Social Worker, Recreational Therapist) with individuals who are living in the Cloverleaf Manor with addiction and mental health concerns. This position may also provide services to individuals with mental health concerns who live in other residential facilities supported by the WISH team. The ILS Worker assist the designated healthcare professional with the implementation of individualized recovery plans by providing goal oriented independent living supports. The ILS Worker relays factual,
timely information utilizing Connect Care which assists in the evaluation and refinement of the overall treatment plan. Supporting clients with transportation needs will also be required (a fleet car will be provided). The ILS Worker meets with the client to assist them in developing / enhancing independent living skills according to the individual service plan. This will entail coaching and / or reminding clients of required daily living skills.
As a Single Independent Living Support (ILS) worker, you will be part of an interprofessional team, assisting patients (client, resident) to develop and maintain skills as identified in the care plan, associated with living independently in the community. You will provide support with preventative health care, health maintenance, quality of life, life skills coaching and connect patients to appropriate community supports. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of Grade 12 or equivalent.
Additional Required Qualifications:
BLS Level C. Non-Violent Crisis Intervention. Two to three years' previous experience with persons with mental illness. Demonstrate understanding, patience and sensitivity when relating to clients and families. Ability to communicate effectively with clients and understand and follow treatment plans and recommendations made by Mental Health Therapist (MHT) or healthcare professionals. Experience with Connect Care considered an asset.
Experience working with mental illness preferred.
