The Toronto Grace Health Centre (TGHC) provides medically complex, specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The services provided at TGHC help facilitate patient flow through the health care system to those individuals who may return to their communities. As of January 2020, TGHC operates in two sites – the Main site (at Church and Bloor Sts) for 119 CCC, Palliative inpatients, the Harbour Light site at Jarvis Street, which houses 28 outpatient beds for patients awaiting full transition into the community,

and the Specialized Care Centre at the Toronto Congress Centre which is currently planned to house 90 outpatient beds by March 2021.



Experienced social worker/discharge planner needed to support with vacation and cross-hospital coverage on an as needed basis

Reporting to the Allied Health Manager, the Social Worker, as a member of the interprofessional team provides safe, quality, patient centered care to individuals with chronic and terminal illnesses including providing support to their families and significant others. The Social Worker maintains a link between the patient, family and community to support the interprofessional care team’s individual patient care plan. This position is a casual contract with the potential to transition into part time.


ü Provide social work coverage and support services to patients/families on differing units depending on current need. This would include inpatient rehab, complex continuing care, post-acute COVID patients and the palliative care program.

ü Provide a range of social work services including psychosocial assessment, crisis intervention, discharge planning and counseling to patients and their families.

ü Act as liaison and assist patients and staff with community resources.

ü Provide comprehensive discharge planning services

ü Deliver in-depth transfer of accountability reports to ensure a seamless transition of patient and family care.


ü Master’s degree or working towards a Master Degree in Social Work from an accredited School of Social Work, or equivalent degree as recognized by the Ontario College of Social Worker is required.

ü Flexible schedule is required.

ü Experience in a hospital or health care setting.

ü Experience with discharge planning is an asset and preferred.

ü Knowledge of the acute, sub-acute and community sector. Knowledge of community resources an asset.

ü Demonstrate ability to welcome change and work within a changing team.

ü Experience in mental health is asset.

ü Strong communicator with proven problem solving and interdisciplinary skills.

ü Member in good standing with the OCSWSSW.

ü Knowledgeable of cultural system perspective that includes family, community, religious and spiritual resources.

ü Excellent interpersonal skills as demonstrated by your ability to develop and maintain positive relationships with patients/families, and members of the health care team.

ü Demonstrated facilitation and problem solving skills.

ü Exceptional time management skills.

ü Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, other healthcare team members and volunteers.

ü Self-motivated, team player, and ability to thrive in a fast-paced environment.

ü High level of integrity, leads by example and exemplifies our Core Organizational Values.
