Job Description



Continuing care is a complex and ever changing environment that requires strong leadership,

creative problem solving, innovation and a commitment to support a resident driven home environment. This position has clinical over-site of a specific site. The services will include accessible programming such as, but not limited to, Adult Day Wellness, temporary community access beds and permanent resident programming. The incumbent works cross functionally and develops collaborative partnerships across the full continuum of care, including external provincial relationships where required. The incumbent provides input and/or expertise as a continuing care representative which includes Committee work as follows: � Quality improvement/accreditation working groups � Occupational Health and Safety � Resident and family council � and others as deemed necessary



Job Qualifications



Required Qualifications





- Licensed and in good standing with professional association and/or regulatory body, if applicable

- Undergraduate degree or diploma in nursing or other allied health profession





Experience

Experience, at an operational level, as a leader who has developed and led innovative planning and programs in a complex, multi-stakeholder environment 5+ years of experience continuing care or formal leadership role, or equivalent combination Valid Class 5 driver�s license

- Travel within the province of Saskatchewan is required

- Where necessary, on-call responsibilities may be required in this position





Additional Information



The successful candidate is responsible to obtain and provide an original Criminal Record Check (CRC) & Vulnerable Sector Check (VSC) from their local police service, RCMP or previous country/countries of residence that is dated within the past six (6) months that is satisfactory to the Saskatchewan Health Authority. Internal candidates may be required to provide a CRC & VSC during recruitment and selection process.
