Job Description





THE LIVEWELL CHRONIC DISEASE MANAGEMENT PROGRAM IS BASED ON A CHRONIC CARE MODEL WHICH INCLUDES EDUCATION AND EXERCISE, DISEASE SPECIFIC MANAGEMENT AND SELF MANAGEMENT. WORKING COLLABORATIVELY WITH INTER-PROFESSIONAL CHRONIC DISEASE MANAGEMENT, PRIMARY HEALTH TEAMS AND COMMUNITY PARTNERS, THE HEALTH EDUCATOR IS RESPONSIBLE FOR THE COORDINATION AND DELIVERY OF PROGRAMS TO ADDRESS SELF MANAGEMENT. THE PROGRAM OFFERS SERVICES FOR MULTIPLE CHRONIC CONDITIONS SUCH AS, BUT NOT LIMITED TO, CARDIOVASCULAR, DIABETES, RESPIRATORY, RHEUMATOLOGY, IBD CLIENTS AND THEIR FAMILIES. THE HEALTH EDUCATOR IS RESPONSIBLE FOR THE OVERALL PLANNING, DEVELOPMENT AND IMPLEMENTATION OF THE LIVEWELL WITH CHRONIC CONDITIONS PROGRAM WITHIN SASKTOON HEALTH REGION IN A VARIETY OF SETTINGS INCLUDING HEALTH REGION FACILITIES AND COMMUNITY LOCATIONS. THE ROLE INCLUDES DEFINING PROGRAM GOALS, DEVELOPING PROGRAM STRATEGIES, MARKETING AND RECRUTIMENT OF PARTICIPANTS AND VOLUNTEER FACILITATORS, IDENTIFICATION OF DELIVERABLES, TIMING AND OVERALL COORDINATION OF PROJECT ACTIVITIES. THE HEALTH EDUCATOR WILL DEVELOP REGISTRATION PROCESSES, IMPLEMENT DATA COLLECTION AND ANALYSIS OF PROGRAM RESULTS, DEVELOP AND COORDINATE A VOLUNTEER BASE (INCLUDING ONGOING SUPPORT AND TRAINING), WORK COLLABORATIVELY WITH THE LWCC TEAM ON MULTIPLE LEVELS, ESTABLISH COMMUNICATION PLANS, AND IDENTIFY RISKS AND CONTINGENCY PLANS SPECIFIC TO THE STANFORD CHRONIC DISEASE SELF MANAGEMENT PROGRAM. THE HEALTH EDUCATOR WILL ENSURE THE QUALITY OF THE PROGRAM AS PER TRAINING AND LICENSING AGREEMENTS, INCLUDING TRAINING OF HEALTHCARE STAFF. A SIGNIFICANT COMPONENT OF THE PROGRAM INVOLVES CONTACT AND A GOOD WORKING RELATIONSHIP WITH NONGOVERNMENTAL AND COMMUNITY GROUPS. THIS POSITION IS RESPONSIBLE FOR THE LIVEWELL WITH CHRONIC CONDITIONS PROGRAM FOR BOTH URBAN AND RURAL SHR SO SOME TRAVEL IS REQUIRED. THIS POSITION REQUIRES SOME EVENING WORK FOR PROGRAM COORDINATION, PROMOTION AND MARKETING



Job Qualifications



Required Qualifications





- Registered or eligible for registration with the appropriate licensing body

- CPR-HCP (Level C)

- A variety of disciplines will be considered for this position based on the following qualifications: Bachelor of Social Work; or Degree or diploma in the following classifications: Registered Psychiatric Nurse, Occupational Therapist, or Physical Therapist; or Speech Language Pathology Masters





Experience

1 YEAR EXPERIENCE IN ADULT EDUCATION AND GROUP FACILITATION PREFERRED INCLUDING TEACHING/COUNSELING, FACILITATING AND LEADING GROUP. SELF MANAGEMENT ACTIVITIES IN PARTICULAR THE LIVEWELL WITH CHRONIC CONDITIONS STANFORD SELF MANAGEMENT PROGRAM. 1 YEAR EXPERIENCE IN PROGRAM/RESOURCE DEVELOPMENT RELATED TO CHRONIC DISEASE PREFERRED. 1 YEAR EXPERIENCE WITH COMMUNITY DEVELOPMENT OR HEALTH PROMOTION, MANAGEMENT ACTIVITIES PREFERRED. 1 YEAR EXPERIENCE WORKING IN A HEALTH RELATED FIELD WITH CLIENTS AND FAMILIES LIVING WITH A CHRONIC DISEASE PREFERRED. 1 YEAR EXPERIENCE WORKING WITH CULTURALLY DIVERSE POPULATIONS IN A COMMUNITY-BASED HEALTH-RELATED PROGRAM PREFERRED.

- Ability to work independently and as a member of a multi-disciplinary team

- Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds

- Ability to competently use computers and software applications as required by the position

- Ability to develop and deliver formal education programs

- Ability to travel both within the region and provincially

- Advanced presentation skills

- Communication skills to transfer knowledge/skills

- Knowledge of community development

- Knowledge of the health region and community resources

CERTIFIED AS A MASTER TRAINER WITH THE LIVEWELL WITH CHRONIC CONDITIONS PROGRAM PREFFERED. ABILITY TO DEMONSTRATE AN UNDERSTANDING OF THE PREVENTION AND MANAGEMENT OF CHRONIC DISEASES. CERTIFICATION IN HEART SAVER HEALTH CARE PROVIDER LEVEL C (CPR) OR WILLINGNESS TO ACQUIRE UPON EMPLOYMENT. KNOWLEDGE OF PRINCIPLES OF SELF-MANAGEMENT AND AN UNDERSTANDING OF BEHAVIOUR CHANGE MODELS. KNOWLEDGE OF THE DETERMINANTS OF HEALTH AND THEIR IMPACT ON HEALTH.
