Unit Summary



The Community and Population Health Services portfolio supports the advancement of integrated health and social services in hospital and into the community.

The Program is responsible for the development and advancement of innovative partnerships with peer, community, academic partners and provides expert leadership in the area of pre- and post-acute care transitions.

The Community and Population Health Services portfolio is accountable for HHS' Population Health, Bundled Care and Alternate Level of Care strategies. The portfolio provides system-wide support through the Site Administrative Managers, Transitional Care & Discharge Specialists, ALC Navigators, Centralized Care and Transition Team, Hospital to Home, Hamilton and Niagara North West Sub-Region Health Links, Integrated Comprehensive Care Program, and Inpatient

Medical Psychiatry Consultation Liaison Teams. HHS Alternate Health Facilities (AHF), McMaster Family Health Team (McMaster Family Practice, Stonechurch Family Health Centre and Canadian Blood Services) and Behavioural Supports Ontario (BSO) are aligned to this portfolio.



Position Summary



Provides administrative support to Community & Population Health Services Program. The Program Secretary has the ability to exercise good judgment in a variety of situations, maintain confidences while balancing multiple priorities including payroll/recruitment, managing calendars, coordinating meetings/events, preparing various materials and documents, attending meetings and recording minutes, monitoring action items and developing related reports.

The Program Secretary serves as a central liaison to the Managers to keep them well informed of upcoming commitments and responsibilities. Integral to providing information and clear communication relative to services, staff, projects and issues within the program. This role requires excellent communication skills to effectively collaborate with directors, managers, staff, health care providers, other administrative assistants and the general public, ensuring strict confidentiality is maintained at all times.



Schedule Work Hours



Days Monday to Friday - hours per day to be determined



This position is temporary to March 2021.



Qualifications



1. Graduate of Community College Business Office Administration program or equivalent

2. Minimum 3-5 years current related experience

3. Demonstrated proficient computer skills in word processing, spreadsheets and database systems, for example: MS Excel, MS Word, MS PowerPoint, Windows 2000, Outlook, Meditech Systems, Taleo, Budget Tool, Management Resource Tool, Eway, Electronic Forms Ordering

4. Proficient keyboarding skills (60 w.p.m.)

5. Demonstrated expertise in minute-taking, transcription and ability in scheduling

6. Demonstrated excellence in effective communication and interpersonal skills within a professional environment

7. Demonstrated effective problem-solving abilities

8. Demonstrated ability to work effectively in a team environment

9. Ability to work on own initiative, organize and establish priorities and able to work in high volume, stressful environment

10. Demonstrated understanding of the corporate context, and ability to make suggestions and create systems which will assist the Program within this context

11. Demonstrated commitment to the HHS vision and continuous quality improvement

12. Demonstrated ability to appreciate confidential nature of position

13. Demonstrated interest in new learning
