The Intake and Treatment Coordinator is an integral member of the interdisciplinary team of the Operational Stress Injury Clinic - Saskatoon Site. The Intake and Treatment Coordinator will facilitate client intake, monitor and support client engagement with the clinic, support processes during the treatment phase, act as liaison with the referral sources, act as a client advocate both within the program and the community and support the client in connecting to community supports during treatment and prior to discharge. The Intake and Treatment Coordinator participates with other team members in ongoing program development, modification and evaluation. The Intake and Treatment Coordinator assumes responsibility for the collection,

distribution and documentation of client information and provides the required follow-up in the active treatment phase.

Job Qualifications

Required Qualifications

Masters Degree in Social Work

Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)

Must have a valid driver�s license. Some assignments may require access to a vehicle for work

Knowledge, Skills and Abilities

Resourcefulness, ability to work independently, and in collaboration with others is required

Knowledge in client case management, chronic pain management and/or vocational rehabilitation (i.e. Career guidance, job retraining, job coaching, vocational assessment).

Must demonstrate excellent interpersonal/interactive skills.

Requires strong concentration skills and attention to detail for formal team assessment, treatment, teaching and report writing.

Requires coping skills to work with clients dealing with stress, role loss and other psychological issues.

Developed skills and abilities in the treatment of clients with operational stress injuries.

Effective oral and written communication skills

Multidimensional thinking and planning is essential

Must be able to plan, organize and coordinate efforts with allied health members as well as communicate effectively and adapt to changing conditions

Ability to perceive the broad scope of the clients involvement in and return to life roles

Responsible for accurate documentation of client related documentation

Requires initiative, sound judgment and excellent organizational skills in order to plan and coordinate information gathering from several sources
