Reporting to the Patient Care Manager, the Patient Care Coordinator will assume responsibility for the coordination of patient care both on a day-to-day basis as well as long-term planning. Also, you will be responsible for identifying the learning needs of nursing staff and patients, providing staff in services on new equipment and/or process change. The successful applicant will need to be familiar with Critical Care standards and initiatives and demonstrate excellence in practice through full role functions: Practitioner, Educator, Consultant, Leader and Researcher.
•Current registration with the College of Nurses of Ontario
•Minimum 5 years of acute care nursing experience in critical care nursing
•Minimum 1 year of charge nurse experience
•Required certifications specific to critical care
•Competent to perform BCLS and ACLS
•Excellent interpersonal and communication skills and the ability to work well with families, patients and all levels of the health care team
•Ability to advocate for quality care based patient outcomes within a multidisciplinary environment
•Ability to work independently and to demonstrate well developed critical thinking and conflict resolution skills
•Evidence of sound organizational and prioritization skills, and the ability to work well in stressful situations
•Experience with staffing, patient flow and patient assignments
•Demonstrate leadership qualities by leading rounds, huddles and initiating new practices and processes
•Experience in the preparation of staffing schedules
•Comfortable with collective agreement language with regards to schedules and calling in staff (ONA, CUPE)
•Proficient in computer skills or willing to learn (Microsoft Office, EZ Schedule, Staffing Request System)
•OHA Leadership Courses (have or to acquire in the first 6 months of hire): Rising Start and Leading Edge
•Current teaching and quality assurance experience required
•Demonstrate participation in quality improvement initiatives (LEAN, QCR)
•Patient Assessment and Health Promotion/Teaching skills
•Must be willing to work with mixed aggregation of patients
•Participation in unit or corporate activities: Preceptor and Mentoring of Staff, Charge Nurse, committees, unit projects and initiatives, and unit representation in corporate committees
•Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety Policy and Legislation.
•Utilize protective equipment provided and identify and reports hazards
•Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
•Excellent work performance and attendance record
•Baccalaureate Degree in Nursing
•Certification in Critical Care Medicine through the Canadian Nurses Association
•Certified nurse in Critical Care through the Canadian Nurses Association
•Enrolled or completed Master's Degree program
•Ability to speak another language
