About the Opportunity





As a Physiotherapist within our transitional care model, you will play a vital role in supporting patients as they regain strength, function, and confidence on their journey from hospital to home. Your work will be grounded in evidence-based practice, compassionate communication, and a strong focus on patient safety and dignity.
In this role, you will:

- Assess, diagnose, and treat patients presenting with actual or potential impairment, pain, functional limitations, or disabilities, using thorough and professional evaluation techniques
- Develop individualized, evidence-based treatment plans in partnership with patients, families, and caregivers, promoting active involvement and honest, goal-focused conversations about recovery
- Implement interventions that support optimal health, functional independence, and safe mobility, while managing pain and minimizing risk
- Collaborate closely and respectfully with interdisciplinary team members to ensure coordinated, patient-centered care
- Maintain an inventory of physiotherapy equipment and supplies, ensuring safe and effective use
- Participate in continuing education to maintain credentials and expand clinical expertise
- Support team excellence by contributing to recruitment, orientation, and training of new team members in alignment with departmental standards and a culture of professionalism and safety
- Foster a positive team environment by supporting professional development, leading team meetings, and encouraging open, respectful communication
- Create and contribute to comprehensive, accurate health records that reflect assessments,



treatment plans, progress, and outcomes
- Instruct patients in the safe use of mobility aids and assistive devices such as wheelchairs, braces, canes, and prosthetics
- Communicate clearly and compassionately — verbally, non-verbally, written, and electronically — with patients, families, substitute decision makers, and members of the interdisciplinary team
- Support a culture of continuous improvement, accountability, and high-quality care
- Perform additional duties as required

About You

You are a compassionate and skilled clinician who values honesty, professionalism, and safe practice. You believe in building meaningful therapeutic relationships and working collaboratively to help patients achieve their highest level of independence and wellbeing.
You bring:

- A Master of Science in Physiotherapy
- Current registration with the provincial College of Physiotherapists
- Minimum of two years’ clinical experience; previous experience in rehabilitation and geriatrics preferred
- Knowledge of the principles, methods, techniques, materials, equipment, and procedures used in physiotherapy and their safe application
- Solid understanding of skeletal, muscular, and neural anatomy and physiology as it relates to physiotherapy practice
- Awareness of potential hazards in physiotherapy treatments and the precautionary measures required to ensure patient and staff safety
- Ability to plan and organize treatment goals and accurately assess patient progress
- Ability to guide and support staff in the proper and safe application of routine physiotherapy treatments
- Previous experience working in long-term care, acute care, and/or a comparable environment considered an asset
- A genuine passion for the healthcare sector and for ensuring seniors and adults in transition have access to high-quality, respectful care
- Ability to provide a clear criminal record and vulnerable sector check upon hire
